It used to be when you uploaded a photo (640×480 let’s say) you could then rezise the photo in the body of the post to fit the design of the post. Now the only choices are Large medium and thumbnail.

How many size options you get when you're about to insert an image depends on the size of the original images in relation to the defaults for those options. You set those defaults in Settings > Media so the size choices you have are self defined. If your main column width is 640 px, you could set the default width for the medium option to about half of that, and the default for the thumbnail option to whatever you prefer. Then you can set the default heights to 0 so that they'll have no effect because usually only the width matters.
For maximum image width by theme see here: http://wpbtips.wordpress.com/2009/07/23/maximum-image-width/

Raincoaster, Have your co-blogger try selecting the visual editor, then saving and going to the editor and reloading the page. If it doesn't show up, repeat this a couple times, switching to the code and saving, then switching to the visual and saving. I had two people who contacted me by my contact form that was able to get it to show up by doing that a couple times.

IE is iffy, and Firefox less so, but still a bit iffy in spots.

By the way, is the blog on .com or is it self-hosted? There have been some pretty enterprising users over at the .org forums that have "fixed" some things with minor code hacks and I'm almost thinking this was one of the things posted.
